Apply for a Grant

 

Requests for proposals are solicited in the spring and fall of each year.

Grant initiatives are subject to change. Availability of the grants and the due date for proposals will be announced via our e-newsletter. Sign-up here or call the foundation office at (508) 879-7625.

Bidders Conference

The foundation will hold a Bidders Conference on Monday, September 24, from 3:00 - 4:30 p.m. at the foundation office to outline funding initiatives and the application process.  Attendance is strongly encouraged but not mandatory.

Proposal Guidelines

Please read the Fall 2018 Grant Guidelines.  The proposal deadline is 4:00 p.m. on Friday, October 19, 2018.  Incomplete and late applications will not be considered.  Applications that are in-progress at 4:00 p.m. on the day of the deadline will not be considered.

Concept Papers

Concept papers and a draft Logic Model are required for all proposals except Continuation Grant funding.  One-page concept papers must be accompanied by a cover sheet and a draft Logic Model and be submitted  by 4:00 p.m. on September 28.  Send to Rebecca Donham at rdonham@mwhealth.org.  Please see the Tips for Writing a Concept Paper and cover sheet below.

Steps to Apply

 Applications must be submitted online. Please follow the steps listed below.  You will be asked to submit general information about your organization and project; a budget and budget narrative; narrative detailing your project; a logic model; and supporting documents.

To start a new application, follow Steps to Apply 1 - 4 below.  You can access the link to submit a new application in Step 4.

Step 1

Download and complete the forms listed and save them to your computer in order to attach them to the online application.

On page 2 of the online application, you will be asked to write a brief abstract of your proposal that summarizes the project including target audience and goals. We suggest that you prepare the abstract prior to proceeding with the online application and save this information to your computer. You will then be able to cut and paste this information from your word processor into the box provided on page 2 of the online application. Please keep your response to 150 words or less. Sample abstract.

You will also be asked to prepare a Logic Model and copy and paste the information into the application. Download a copy of the Logic Model [DOC]. Do not upload the document itself.

Step 3:

On Page 3 of the application you will be asked to upload the following supporting documents, if applicable, to your online application. Documents should be in Word, Excel or PDF format, and must be uploaded through the online system. Do not upload files with JPEG, JPG, or TIF extensions. Documents will NOT be accepted via U.S. mail, email or fax.

  • List of your organization's current Board of Directors
    List the name of each director, town of residence and affiliation only. Not applicable for municipal agencies. Do not include contact information such as emails and phone numbers.
  • Financial Statements for your organization's most recent completed fiscal year, including balance sheet and income and expense statement.
    If your organization is audited, please provide a complete set of audited financial statements. Not applicable for municipal agencies. Otherwise, submit a copy of your 990.
  • Letters of Collaboration
    Letters of Collaboration are not required, but highly recommended. The letters should be signed and specifically state the collaborator's individual or organizational contribution to the project. Replicated form letters from multiple organizations are discouraged. Multiple letters must be scanned into one document.

Note: Proposals involving schools must include a letter of collaboration from the superintendent.

Complete the online application. The online application consists of four areas:

  • Applicant Login
  • General Organizational/Project Information Page
  • Proposal Abstract and Logic Model
  • Attachment Upload Page

Important Notes: Applicants must create an account in order to access the application and save their work for completion at a later time. This user account and password will allow you to submit your online application and your grant reports, if your project is funded. Therefore, we highly recommend that your account information and grant application are created and submitted by the project director or development director. You will receive an email with a link to your application and confirming your password.

A Concept Paper [DOC] must be submitted for all proposals except Continuation Grant requests before proceeding with the Online Application.
