Health Equity Award

The MetroWest Health Foundation annually invites nominations for its Health Equity Award.

The MetroWest region is one of the healthiest regions in one of the healthiest states in the country.  Yet inequities still exist based on race/ethnicity as well as economic status and zip code.  There are many in our community working everyday for equity.  The Health Equity Leadership Award recognizes an individual or organization that has had a significant impact on addressing health inequities in the region.

Who is eligible?

An individual, nonprofit organization, municipal entity, or coalition that meets the following criteria:

·         Individuals must be employees or volunteers of a MetroWest organization, coalition or municipality

·         Organizations, coalitions and municipal entities must be physically located and conduct the majority of their work in MetroWest

·         The MetroWest region is defined as the towns the foundation serves.

Who should be nominated?

The award will go to an individual or organization that have achieved one or more of the following:

·         Implemented a systems-change approach to improve health outcomes for those most affected by health inequities

·         Promoted more equitable access to high quality health care

·         Influenced public policy in a way that promoted health equity

·         Changed internal systems to create greater institutional equity

·         Promoted greater understanding and awareness of health inequities in MetroWest

·         Successfully engaged populations most affected by health inequities to improve health outcomes

What is the nomination and selection process?

·         Individuals and organizations must be nominated by someone other than themselves

·         The award winner will be determined by a sub-committee of the MetroWest Racial and Ethnic Disparities Workgroup, which is composed of leaders from a variety of MetroWest agencies

What is the timeline?

·         Nominations are due by February 22, 2019
